Role of Health Care Professionals in Home Care

Your physician is usually the leader of an interdisciplinary healthcare team that may include nurses, therapists (such as: speech, physical, occupational, and respiratory therapists), social workers, personal care aides, home medical equipment suppliers, and most importantly, formal and informal caregivers (e.g., family members). Although all team members are important, the physician is legally responsible for working with you or someone you identify to determine your health care needs. Physicians can also develop, certify, and recertify the plan of care.

It can enable safety, security, and increased independence; it can ease management of an ongoing medical condition; it can help avoid unnecessary hospitalization; it can aid with recovery after an illness, injury, or hospital stay—all through care given in the comfort and familiarity of home. Home care can include:

  • Help with daily activities such as dressing and bathing

  • Assistance with safely managing tasks around the house

  • Companionship

  • Therapy and rehabilitative services

  • Short- or long-term nursing care for an illness, disease, or disability—including tracheotomy and ventilator care.

  • Bathing, grooming, using the toilet, and dressing

Care is available up to 24 hours a day, 7 days a week, including the option for live-in help. We will work closely with you and your family to determine the best care schedule to meet your needs.
